Aurora Health at Home has been providing home care with compassion, professionalism, and clinical excellence for over 100 years. We serve individuals of all ages throughout Northeastern Wisconsin.

Our goal is to provide health care solutions that enable people to remain independent within their communities.

Aurora Health at Home Services

Aurora Health at Home is the most comprehensive source of health home care services in Northeastern Wisconsin. Our services include:

  • Skilled nursing, rehabilitation therapy, home infusion services, home health aides and more
  • Home medical equipment and supplies provision, including delivery and set-up
  • Palliative care - comfort and quality of life care at any stage of disease
  • Hospice care - end of life care in-home or at assisted living, nursing home, or hospice sites
  • Telehealth

  • Have been hospitalized
  • Have had surgery
  • Have serious or multiple health problems
  • Suffer from a chronic disease
  • Are disabled, living alone or with a caregiver
  • Need assistance to remain at home – regardless of age, from newborn to elderly

An Aurora Health at Home representative conducts an in-home assessment to help you establish your care plan.
