Aurora Health at Home Services
Aurora Health at Home is the most comprehensive source of health home care services in Northeastern Wisconsin. Our services include:
- Skilled nursing, rehabilitation therapy, home infusion services, home health aides and more
- Home medical equipment and supplies provision, including delivery and set-up
- Palliative care - comfort and quality of life care at any stage of disease
- Hospice care - end of life care in-home or at assisted living, nursing home, or hospice sites
- Telehealth

- Have been hospitalized
- Have had surgery
- Have serious or multiple health problems
- Suffer from a chronic disease
- Are disabled, living alone or with a caregiver
