The Aurora Health at Home hospice team provides care and comfort to you and your family as the end of life approaches.

Hospice is a concept of care, not a place of care. Hospice affirms life and maintains the dignity of each individual we serve. Hospice focuses on maintaining quality of life as the end of life approaches.
While the Aurora Health at Home hospice staff works primarily with families in private homes, we will follow you elsewhere as needed—to an assisted living center, nursing home, or hospital. We work with you, your family, and physician to develop a plan for appropriate care.
Holistic Care
We strive to provide pain-free comfort while attending to you and your family’s spiritual, emotional, social and physical needs.
Your hospice team includes physicians, nurses, social workers, clergy, aides and highly trained volunteers. We understand the physical care needed by people with many life-limiting conditions such as cancer, or end-stage heart, lung, liver, or kidney disease, AIDS, Alzheimer’s disease and dementia.
We also understand the emotional needs you and your loved ones may have as you face the end of life. We will help you experience peace and healing.
Home Hospice Care
Most individuals and families seek hospice care in their homes where a primary caregiver (typically a family member or friend) provides care with the assistance of our hospice team.
As with hospice care in any type of facility, our caregivers visit your home regularly to assess, consult and counsel, as well as to direct and implement the hospice plan of care. We will assess your needs and provide necessary supplies and equipment needed for hospice care. These supplies may include a hospital bed, wheelchair and oxygen.
We also provide medications necessary for comfort and symptom management as they relate to your primary hospice diagnosis. If you or your loved one is not able to remain at home, we work with you to find appropriate accommodations in a local extended care facility, or hospice inpatient facility where available.
Continuous Care
If you have increased medical needs for acute symptom management, continuous home care may be available for a limited period.
Respite Care
Respite care stays up to five days long are available when you, as the caregiver, need relief from round-the-clock care giving responsibilities.
Bereavement Care
Bereavement is the time of mourning after a loss. We will provide support for surviving loved ones to help you through the grieving process.
Volunteer
Hospice volunteers provide compassion and companionship. They assist with simple household tasks or simply provide the connection of good conversation.
