Aurora BayCare Selected as Patient Navigator Program - Focus MI Diplomat Hospital

Aurora BayCare Medical Center is one of 15 hospitals from across the country selected to be a diplomat hospital in the American College of Cardiology (ACC) patient navigator program focus MI.

The program will provide hospitals participating in the ACC's NCDR ACTION Registry the ability to have access to evidence-based toolkits, a self-assessment tool, a community of national experts from leading hospitals and institutions and assistance with creating a patient-centered, team-based approach to improving care for Acute Myocardial Infarction (AMI) patients. In addition, the hospitals will focus on identifying best practices for improving patient care and reducing readmissions.

Aurora BayCare kicked off the program on Tuesday, March 27, with a presentation and celebration. Aurora BayCare physicians, caregivers from the hospital's Acute Myocardial Infarction (AMI) Committee and representatives from Astra Zeneca and the ACC were in attendance.

The patient navigator program focus MI is an expansion of the original patient navigator program launched in 2013, of which Aurora BayCare was one of the 35 pioneering hospitals chosen to provide personalized services to heart failure and AMI disease patients and help them avoid a quick return to the hospital.
